These are the forms and procedures that we use on every one of our outreaches.  Some are from Revival Ministries International (RMI) but most of the forms and instructions has been personnally developed or modified over years of outreaches.  Please feel free to use them as you will or edit them yourself or you are welcome to contact me and I would be glad to assist you in personalizing them for your church or ministry.  It is all about souls and eternity!  Whatever it takes lets win the lost at any cost!  I look forward to hearing from you.  Thank you for making an eternal difference.

PASTORS INTRODUCTORY LETTER

Thank you for your consideration in hosting evangelistic outreaches. We are available to train, equip and lead an outreach at your church at no extra work, cost or time on yourself. At a scheduled time we will offer to those who would like to join us the option and opportunity to attend a short training and then to go out on the streets somewhere around your church! Our goal is to flawlessly work with you in setting this up. And at no extra charge, we will provide all of the outreach materials needed which which includes a flyer that highlights your church. From those who pray the prayer of salvation we will provide you with the prayer sheets for follow up and discipleship. The whole operation will take 6 hours, from 9am at arrival to 3pm when everything is completed. Our greatest desire is to establish an outreach team within your church and for your members to reach many souls for Jesus.

GETTING STARTED

At your discretion the outreaches can be scheduled weekly, biweekly or monthly. I am available to assist you with every outreach. My goal is to identify and work with those who go out on the outreaches who are able and called to lead evangelistic outreaches independently. I will be physically there during the first outreaches and after this I will be available by phone or email. Consistency is the key, I will stay in touch with leadership, answering questions and working through any issues that might arise.

RESPONSIBILITIES OF THOSE WHO PARTICIPATE

To accomplish the great commission we must be unified and be available to minister to one another. Our job here on earth is to preach the good news of Jesus, release people from the clutches of the kingdom of darkness through the Gospel of Jesus Christ, connect them to the Father heart of God and be an eyewitness of heaven reaching out for that one lost sheep! The following are responsibilities to help the team walk in unity. Every first-time attendee will sign a statement of agreement, release from liability and provide their contact information.for future outreaches which will be turned over to the leader of the outreach.

PROFICIENCY LEVELS

Being of a military background I was tested, trained and measured through proficiency guidelines. Each person on the outreach team will be trained on individual tasks which are clearly defined, observable, and measurable establishing a solid foundation.. Tasks enable Harvesters to master the necessary skills.and move onto to the next level.

This is the training part for the outreach which is completed before every outreach.

HARVESTER AGREEMENT FORM

Shared and used in training sessions. It includes an agreement, responsibilities, release of liability and contact information. This section is 6 pages long and each first-time participant will receive a copy. The last 2 pages are the signature page; once signed, it is kept by the outreach director and remaining pages are given to new participants. Make sure you have plenty of copies of this section on hand for new visitors and as a training tool.

YOUR CHURCH FLYER

Included is a worksheet that provides us with the needed information to put together an outreach flyer for your church for this revival absolutely at no cost! We can only provide a certain amount of flyers for the revival but will provide a digital PDF file of your church flyer by email so that additional outreach fliers can be printed when needed. On the back of the flyer is the Gospel Soul-Winning Script and the Spanish Survey, which is a great tool if you’re not fluent in Spanish or you can include your own announcements. Present this to a Spanish speaking person and watch incredibly as they lead themselves to the Lord. If they select “Si” on the last question, have them print their name on the prayer card and then pass the information along to a local Spanish ministry.

PRAYER CARDS

This is the heart and soul of the ministry - people! In the top box the Harvester puts his/her name, leader’s name, area visited and a soul goal which is how many people they are believing to pray the prayer of salvation and this also includes the total number of souls won on that outreach for that Harvester.  In the body of the form it tracks Salvation, Rededication, Church Commitment, Prayer Request, Youth and Hispanic.  The prayer card includes notes which can be used for special prayer requests.  It also allows you to confirm this person's permission to be contacted.

TESTIMONY CARDS

If something spectacular happens (which it always does) the Harvester fills out this card. These are shared with the outreach leader which is shared with the Pastor if it is notable.

OUTREACH TEAM STAT SHEET

Each outreach will have at least one team leader.  The larger the group the more team leaders that will be needed, this person will most likely be an outreach leader.  Each stat sheet records who went out, phone numbers, how many salvations each person had and the area visited.  On the back of the sheet are detailed instructions on what to do before, during and after the outreach.

AFTER THE OUTREACH

WEEKLY SOUL REPORT CARDS

The Evangelism Director provides statistics with the Pastor weekly of how many salvations and how many laborers, locations visited and next scheduled outreach is. This is an excellent tool to be shared with the congregation to rejoice together over the people who have been born again. This will also promote excitement and increase participation and support of church members. Your church will never be the same again!

1. Upon completion of each outreach 3 copies of every Prayer Card will be provided to:

   1. The Church’s Intercessory Prayer Person

   2. The Evangelism Follow-up Team

   3. The Church Administrator

2. Church’s Intercessory Prayer Person:

   1. i. Salvation, Healing & Deliverance

   2. ii. Any special prayer needs noted on the prayer card

   3. iii. Pray for individuals throughout the year

   4. We may just be planting seeds

3. Phone Call Follow-up:

   1. Follow-up by phone within 48 hours after the conversion

      1. Use the Follow-up Phone Call Script

      2. Verify contact information including email address on the Follow-Up Phone Call Report Card

      3. Pray for any needs

   2. 3 Week Follow-up Call

      1. How they are doing & have they found a place of worship

      2. Pray for any prayer requests

4. Church Administration:

   1. Mail out Pastor’s Welcome Letter within 3 days

   2. Email new believers within 7 days

      1. Encourage them to attend the Church

      2. Encourage them to attend the Discipleship Training Class

   3. Keep in touch through email if they have not been involved with anything and continue to invite them to your events
